*The 5% SERVICE FEE covers the cost of wear and tear on electrical equipment, product usage and replacement, tools, applicators, brushes, cleaning supplies, kit cleaning and restocking, hourly office expenses and supplies. There is no additional fee for using any of the listed Payment Options in this agreement. ** The GRAND TOTAL is to include a 5% SERVICE FEE ***The 50% DEPOSIT is half the amount of the GRAND TOTAL

Gratuities can be paid by venmo, check, credit card, or Bank of America deposit, with the final bill or in cash to individual artists. Please feel free to add into your payment or to discuss bridesmaids/attendants paying their stylist individually.

* I confirm that by returning or emailing this form in part or in full it is considered an agreement between both parties (signed or unsigned) as approved by Go-Diva LLC and/or Margarita Potts. In addition, I hereby confirm that I understand and accept that by paying any amount including, but not limited to the 50% required deposit via cash, personal or cashier's check, money order, bank transfer, credit card or barter agreement as a deposit to reserve dates, times and/or appointments it will be considered an acknowledgement and acceptance to the terms in the Rates & Services Agreement and the I Do Hair and Makeup Artistry LLC/Go-Diva LLC Policy Agreement between both parties. The remaining payment is then due in full by the day of the ceremony. 

 
 

 

I Do Hair and Makeup Artistry LLC/Go-Diva LLC Policy Agreement  
 
The Contract/Agreement will be signed or returned by the person booking services for a particular session, and will specify the location, date, time, number of persons receiving makeup/hair service, and what time each person will have reserved. If services for more than one person are being booked, all deposits must be paid at the time of contract signing to guarantee each reservation. Due to the possibility of schedule conflicts, additional makeups/hair dos added after contract signing or changes made to the location, date, and time of event will be done at the sole discretion of I Do Hair and Makeup Artistry LLC/ Go-Diva LLC and/or Margarita Potts. This contract is the only agreement between the person (booking or contracting the services of I Do Hair and Makeup Artistry LLC/Go-Diva LLC (hereafter referred to as clients) and/or Margarita Potts.  
 
BOOKING APPOINTMENTS & TIMELINES:  I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts & or any of its Freelance/sub-contracted Artist books on a first come first serve basis. No appointments are (A) considered officially booked until the required 50% deposit of services requested has been made by the client. (B) The IDHMA Rates and Service Forms have been filled out and returned.  Agreement will contain a start time and end time as a general guideline. Each service requires a certain length of time established by I DO HAIR AND MAKEUP ARTISTRY to be finished by. This time is not to exceed the time limit provided by I Do Hair and Makeup Artistry LLC/ Go-Diva LLC and/or Margarita Potts (each agreement time will be discussed on a case to case basis and the above times ARE GENERAL until further secured at trial or prior to wedding date, once schedule has been made it is up to the the discretion of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts to adjust or change any timeline after timelines have been finalized). Appointment times may be amended/discussed via text & or email after agreement has been made). Since I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, books on a first come first serve basis they cannot base clients appointment times off third party timelines (ie wedding planners photographers etc * see Third Party Clause below). When reserving your date/times, please be sure to book accordingly. Any additional needs outside agreement will only be performed at the discretion of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts. All persons involved in appointments need to be available at the scheduled time of said appointment in order to not break the agreement. All services for more than one person must be at the same location and consecutive in time (no gaps in between). Due to circumstances outside our control artist may vary. Veil placement will be made by Artist AT TIME OF appointment not passed appointment time - i.e. BEFORE dress and photographer etc. (*see Veil Placement Clause below). Artist are not able to stay passed appointment times for veil placement due to other appointments in our schedule, however Artist can show bridesmaid/bridesmaid/parent how to place veil and remove if needed. Once Artist have completed originally booked services and there are no additional services to be completed I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts & or any of its Freelance/sub-contracted Artist are required to leave regardless of "done time" in agreement.    
 
DESTINATION WEDDINGS: Weddings prices packages and travel will be discussed on a case by case basis  
 
EARLY CALL TIME: A $50-100 per hour fee will be charged for appointments before 8:00 am.  
 
HOLIDAY/HOLIDAY WEEKENDS: An additional 100.00 will be added to final billing for Holiday and/or Holiday weekend (if Holiday fell during the week)  
 

VEIL PLACEMENT: Veil Placement service can be added for a 50.00 fee. Veil Placement will be done within 5 minute of artist completing Bride's Hair and Makeup. Due to our tight schedules Veil Placement appt that have been book will incur a late fee of $50.00 for every 10 minutes delay. After 10 minutes appt is not guaranteed if Bride is running behind in getting dressed. Due to circumstances outside our control artist may vary. Veil placement will be made by Artist AT TIME OF appointment not passed appointment time - i.e. BEFORE dress and photographer etc. Artist are not able to stay passed appointment times for veil placement due to other appointments in our schedule, however Artist can show bridesmaid/bridesmaid/parent how to place veil and remove if needed. 
 
ADDITIONAL SERVICE TIMELINE: For Grooms Attendants Family members etc receiving services outside of the wedding party or any services not performed in the same location and the room as the Bridal Party a 50.00 fee will be charged. Clients receiving additional services must be present and ready in room to receive their services 1 1/2 hours prior to their projected timeline. Due to our tight schedules a late fee of $50.00 will be charged for every 30 minutes of delay &/or is not guaranteed if Clients are not present or are running behind for any reason.  
 
DELAYS: A late fee of $50.00 will be charged for every 30 minutes of delay when a client is late for the scheduled time, or if scheduled time exceeds allotted time because of client delays. After 45 min delay appointment will be cancelled and all deposit monies are non-refunded and balance will be due in full.  
 
DOUBLE BOOKING CLAUSE: Due to the high volume/demands during bridal seasons Go-Diva LLC and/or Margarita Potts may have multiple bookings/events per day (just as a salon would) therefore appointments time slots and scheduling will be discussed prior to the clients event to allow ample time per each wedding/event. It is imperative to avoid potential scheduling conflicts, by having ALL clients/attendant/bridesmaid available for their appointed booking time during event. Should client/attendant/bridesmaid be more than 30 minutes late services may be cancelled (at the sole discretion of the artist) and all deposit monies are non-refunded and total balance will be due.  
 
SATISFACTION GUARANTEED: In order for any Makeup/Hair and overall look to be guaranteed the services/looks shall be determined PRIOR to application with a "Reasonableness Test" by booking a run thru. Reasonable effort by artist will be made during appointment. Appointment time is not to exceed allotted application time - unless approved prior by I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts. Acceptance of completed makeup/hair by client is acknowledgement that service(s) has been done to his/her satisfaction. Ample time is given for each service upon booking. However artist will not be held responsible for unfinished Makeup/Hair due to late client or for "suggestive satisfaction" or for unrealistic expectations. Expectation example: client provides photo of model with different hair color, length, texture, skin tone & texture, eye color etc - reasonable effort will be made to get as close to the overall look as possible.  Artist(s) cannot be responsible for any makeup/hair issues after artist(s) have left.  This includes veil placement/removal client adding makeup to their face, lipstick wearing off etc. Clients requesting use of any of their personal beauty product releases artist of any satisfactory guarantee service.   
 
TRIAL MAKEUP/HAIR STYLING: Makeup/Hair Trials are (normally) scheduled Tues-Fri - due to the large volume of weddings & events on weekends. Some exceptions for weekend trial/run-thru can be made if available. I Do Hair and Makeup Artistry LLC/ Go-Diva. A makeup/hair trial is available for services that do not include one. The additional trial makeup service costs at a variable and standard rate. It is limited to 3 hour. If you are more than 15 minutes late for your appointment, it may be cancelled and a late fee of $25 will be applied to the final bill. Please call at least two (2) days in advance to reschedule a trial makeup. Trials are separate, service/fee and trial cost is due prior or when trial service is rendered. Please be advised I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts contractual agreements with TV, film and print clients, last minute (contractual) filming may need to take precedence over a trial/run-thru. A new trial date and time will be worked out between both parties. Please allow ample time for trials/run-thru (approx. 1 and a half hours or more per service). Please be prepared with images, looks, and ideas. An image or internet photo of how client normally wears hair and makeup for a night out. This gives stylist an idea of what client is most comfortable wearing i.e. mascara eyeliner and what side the part hair etc. *Please see Travel Clause for travel fees outside of San Juan area.  
 
LEAD & ALTERNATIVE ARTIST: In an extreme/unforeseen circumstance such as an "Acts of God", accident, illness, etc I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts reserves the right to have one of their alternative artist perform services in place or prior scheduled artist.  In most cases the Lead Artist will work on Bride. If time permits Lead Artist will be perform services on attendants scheduled. Please note Lead Artist may have multiple weddings/booking on same date therefore the I Do Hair and Makeup Artistry LLC/Go-Diva LLC stylist will work on Bridal party.   
 
 

ADDITIONAL ARTIST:  I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts has a standard procedure when booking and scheduling artist. In the cases that client's needs, timelines, or bridal party is greater "the norm" in terms of amount of services an additional artist fee of 100.00 per additional artist may be required. Example: 15 people needing to be done in 2 hours an additional artist(s) fee would be required.  
 
BRIDEZILLA/MADONNA CLAUSE: I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its freelancers, employees, and agents reserves the right to cancel your agreement at any time for overly rude, or obnoxious behavior from anyone in party including Bride, Bridesmaids/Attendants, Mother of Bride, family members and guest. Any comment thought to be overly rude or obnoxious behavior will merit a warning; any further comments will result in the termination of services at no expense of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents. In this case, clients will be required to pay any/all expenses accrued. * But not to worry - we are pretty open-minded.  
 
BOOKINGS: To secure a date/time slots a 50% deposit of all services to include the 5% service fee is due at the time of "returning" contract/agreement. The deposit is non-refundable and non-transferable. Please be advised, dates and scheduled makeup times will only be reserved when contract/agreement and deposit is received. These times are subject to change at the digression of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts. I Do Hair and Makeup Artistry LLC/Go-Diva LLC artist/assistants may be performing services on Bridal Party. In order to exclusively secure the services of Margarita Potts for bridal party/with a booking of only your wedding on your wedding date a minimum 500.00 day rate (5 hours) will be applied to service total.  
 
PEAK SEASON: Due to heavy volume Margarita Potts/IDo does not personally take the "Bride Only Package" during peak bridal season's. There are a minimum number of services necessary to book during this time. Exceptions will be made on a case by case basis so please inquire. If less than 5 hair and makeup services are granted during peak season the I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts team will schedule morning or late afternoon time slot in order to accommodate other weddings/events as well.  
 
BRIDAL PLANNERS/THIRD PARTY TIMELINES: I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts do not use third party timeline. Agreement of wedding times is determined between client and I Do Hair and Makeup Artistry LLC/Go-Diva LLC Margarita Potts prior to wedding date. I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts times will be given to wedding planner and photographer (by bride) for them to plan accordingly. Times are subject to change.  
 
WEDDING PLANNER/THIRD Party: In the event of a wedding planner or third party preparing and/or securing vendors for their client, the wedding planner or third party will be held liable and in agreement with this document in its entirety, including but not limited to any and all fees, losses and damages, etc. Margarita Go-Diva & I DO HAIR AND MAKEUP ARTISTRY work hard to provide clients with their desired timeline and book on a first come, first served basis which can only guarantee the desired timeline to the first booked event/wedding. It is the responsibility of the wedding planner or third party to request information regarding any other events booked on their wedding/event date. In such cases where a wedding planner or third party bears liability for their client and signs, either physically or electronically returning, this agreement, it is understood and accepted that a 20% Gratuity will be included in the Total Balance and under no circumstance will be removed.  
 
FORCE MAJEURE/UNFORSEEN CIRCUMSTANCES: In an unforeseen circumstance such as extreme weather, flood, hurricane, earthquake, war, invasion, military action, acts, orders, civil disturbance, riots, strikes, lock outs, trade disputes causing cessation or slow-down of work, or inability to procure services or material after due and timely diligence, late deliveries or late performances of services from sub-contractors or suppliers, breakdown of machinery, fire, flood, death, accident or any cause beyond the reasonable control, making performance inadvisable, commercially impracticable, illegal, or impossible I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts & or any of its Freelance/sub-contracted Artist will be exempt from being required to carry out the agreement and will not be liable to preform future services.   
 

 
NON DISPARAGEMENT: No negative disparaging comments will be posted by any client on any web or social media site message boards or via internet including Facebook Wedding Wire & The Knot etc. This will result in court proceedings to recover any damages to Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees, and agents. Client will also be responsible for all attorney fee.  
 
USE OF IMAGE: Photo credit for all photos in any place, website, marketing materials etc. will be granted/given as such: Makeup (and/or Hair) by I Do Hair and Makeup Artistry LLC/Go-Diva Onsite Bridal Beauty &/or by Margarita Potts www.MargaritaGoDiva.net or www.IDoHairandMakeupArtistry.com. I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees, and agents may use on it's website - and/or in any brochure, flyer or other advertising it deems necessary - any and all photographs, video, audio, and any other digitally or chemically stored media that is captured or recorded by I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees, and agents are sole property of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and Margarita Potts. Client(s) agree to release any and all claims regarding use of his/her image for such purposes. Client(s) also agrees to release name and contact information of professional photographers and/or videographers used for recording any event for which Go-Diva LLC has been contracted to do makeup. Client(s) agree to release to I Do Hair and Makeup Artistry LLC/Go-Diva LLC use of said photographers/videographer's photographs and recordings. This signed agreement serves as a release to client(s) photographer/videographer and authorizes them to release to I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its, employees, and agents and photographs/recordings to use on any promotional materials I Do Hair and Makeup Artistry LLC/Go-Diva LLC deems necessary.  
 
BRIDAL/CLIENT RELEASE FORM: For good and valuable consideration herein acknowledge as received, I hereby grant to Margarita Potts (hair, makeup artist, and Photographer of I Do Hair and Makeup Artistry LLC/Go-Diva LLC) and those acting with her authority and permission, the right and permission to use and publish photographs of me for the purpose of promotion in print and on the web. I hereby waive any right that may have to inspect or approve the final product. I hereby release Margarita Potts (hair, makeup artist, and photographer) and those acting with her authority and permission from any liability that may occur as a result of publication of said photographs, including without limitation any claims for libel or violation of any right of publicity or privacy. I hereby warrant that I have the right to contract in my own name. I have read the above release, and understand and am fully familiar with its contents. This release shall be binding upon me and my heirs, legal representatives and assigns.  
 
WEDDING DAY SERVICE CANCELLATION: All monies (except for deposit) will be refunded if contract is cancelled within one (1) days (24 hours) of contract signing. After the one (1) day grace period, should a client cancel the booked event or any service on a contract, the payments cannot be refunded or transferred and the remaining balance shall be due in FULL within 7 days. Should I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its, employees, and agents have to cancel services for wedding or event (this does not include trial run thru) 100% of the deposit paid will be refunded by check within thirty (30) days. Client agrees that the refund of 100% of the deposit is the only liability to I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its, employees, and agents and no other expenses to them will be accrued.  
 
SERVICE LOCATION AND REQUIREMENTS: Location of service for the day-of-event will be at the discretion of the client, but there are certain requirements the makeup artist needs to complete the makeups. A "set up" table/work area needs to be made available for the makeup artist at said location. Working electrical outlets must also be made available for use by makeup artist. Ample lighting, whether by means of natural light or by lamps, is necessary for services to be performed properly. A chair is requested. Climate controlled area for makeup must be granted at all time.  
 
ADDITIONAL FEES, LOSS AND DAMAGES: Where parking, valet or toll fees may be accrued; the amount will be included with the final bill and due for payment on the day of the event. Except as otherwise specifically provided in this Agreement, any and all other incurred expenses including, but not limited to, local transportation costs (rental or leasing of vehicles and repair and maintenance of same), shop rental, utilities, insurance, rental, repair, and maintenance of tools and equipment and any and all other costs, fixed or otherwise, required by I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents. Pleases note that kits will be in need to be in a controlled temperature/environment at all times. Loss and/or damages will be billed to the client at I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents at full price of product used taken lost or damaged.    
 
DESTINATION WEDDINGS: Weddings, prices, packages and travel will be discussed on a case by case basis.   
 
TRAVEL FEE/TIMES: A travel fee will be assessed on a case by case basis. Should travel fees be needed  for locations outside of the city limits at a variable and standard rate of 49.5 cents per mile. The amount will be determined at time of inquiry/agreement negotiations. These locations are subject to appointment times set by IDHMA due to multiple bookings, travel time, requested artists, etc. All travel will be discussed on a case by case basis.  Estimations: including parking, gas, tolls, bellmen assistance, cab rides, car rentals, travel time, etc. Should a start time or travel times be needed before 7:30am a room shall be provided for the team the night before services/events at the hair and makeup location. If hair and makeup location is a private residence hotel accommodations will need to be discussed.  
 
AIRFARE AND ACCOMMODATIONS: Any additional travel cost such as mandatory airfare for travel for an booked are to be paid by client(s). Costs may include, but are not limited to: drive/travel, airfare, hotel, transportation, parking, per diem, service incidentals and all taxes and/or fees. A minimum charge requirement will be discussed on a case by case basis for any and/or all out of metro area. 
 
TOUCH UP/WAITING FEES: For multiple changes or looks a hotel room shall be provided for the team for “day  of holding” (waiting) for the next hair and makeup touch ups or changes. A wait fee of $120 within the first hour will be added for any scheduled hold/wait appointment. An addition fee $25 per every 15 mins thereafter.  
 
INDIAN ASIAN ARABIC & MIDDLE EAST CULTURAL WEDDINGS: We charge for all incurred parking expenses. We do not accept a bride without a trial or, if you forfeit your trial the cost on your wedding day is double* Junior bridesmaids may be considered one of these services at full price – once 5 services are reached their prices may go down, please ask for a quote. Travel fees are applied outside of city limits or on bookings of less that 3 services. Tips are not included in the price. There is an early morning fee of $200 for weddings before 7:30am. For multiple events in one day a separate room is required and will be made/booked & available to artists for rest in between events. If a room is not provided there is a return fee of $150+ for artist(s) leaving and returning to the bridal party/attendants for touch ups (for the next ceremonies or event scheduled).  
 
ASSISTANT/CHAPERONE CLAUSE: I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents are allowed to enforce reasonable precautions and background "screening" of all potential clients prior to and during all events and makeup applications. I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts and/or any and all of its employees and agents reserve the right to bring a chaperone or assistant to any event. This is used for safety purposes for the Artist. Should there be any unsafe or uneasy feelings with a client or their guest I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents reserves the right to termination the services and no expense of I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents. In this se, clients will be required to pay any/all expenses accrued.  
 
LIABILITY: Any skin condition including dehydrated skin should be reported by the client to the makeup artist prior to application and, if need be, a prearranged trial or sample test of makeup may be performed on the skin to test reaction. Client(s) agree to release the makeup artist, I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents from liability for any skin complications due to allergic or rare reactions. It is the responsibility of client to provide special makeup or products which they require for allergy reasons or sanitation purposes/needs. I.e. personal mascara & lipstick. I Do Hair and Makeup Artistry LLC/Go-Diva LLC and/or Margarita Potts, and/or any and all of its employees and agents cannot guarantee the longevity of the over-all look when using the clients makeup, hair or beauty products. CIRCUMSTANCES BEYOND OUR CONTROL: In the event of a power outage, broken machine defect in applicators etc., makeup artist will not be held responsible for airbrushed makeup or any service not being performed. Makeup will be applied manually by brush/sponge due to the circumstances. The deposit will not be refunded and the balance will still be due.  
 
GRATUITIES: Gratuities are NOT INCLUDED in the pricing lists. 20% Gratuities (of all services performed) will be added to final payment for clients with 5 or more services or for clients using event/wedding planner. For Agreements containing 4 or less suggested gratuity is 20% (of all services performed). PAYMENT: The FINAL balance is due on the day of the event as one payment - no exceptions. The person(s) responsible for the entire balance of payment is the person(s) who has signed the booking agreement. Acceptable forms of payment are: cash, check, and venmo payments made payable to Margarita Potts. Any final balance not paid by the day of wedding will incur a $100.00 late fee PER day.   
 
 
SEVERABILITY: If any provision or provisions of this Agreement shall be held to be invalid, illegal, unenforceable or in conflict with the law of any jurisdiction, the validity, legality and enforceability of the remaining provisions shall not in any way be affected or impaired thereby.  
 
RETURNED CHECK: If any check is returned by the bank for non-sufficient funds (NSF) a $30 fee will be charged on top of bank fees and 100% of all fees contracted. Any booking made under such circumstances, (NSF), is subject to cancellation and any deposits paid will be forfeited.  
 
ONE PARTY CHECK: One check per Bridal party. Cash will always be accepted however multiple/split checks are not allowed from 3rd party.  
 
MINIMUM: A minimum Grand Total cost for any and all requested services may be required in accordance with but not limited to Bridal Seasons, the availability of the IDHMA team, location, desired timelines, etc.  This minimum will be determined on a case by case basis. There is a minimum charge of $450 for an onlocation wedding service located within metro area.  For wedding services located outside of metro area, a minimum number of services are required.  These locations must include 5 services in addition to the bride to total an amount of $875 or more.  *Please inquire as minimums may change depending on popularity of the date of your event

Rates are subject to change without notice prior to signing contract. Call for current rates as of today’s date. Once you have signed a contract with the current rates, you are guaranteed that price if the rates go up. Rates are per person or the Artist’s minimum, whichever comes first. Client is responsible for all parking, toll, and travel fees. Payment in full is expected day of services provided. Additional people will be charged at time services are rendered.     
    
*I agree to all terms set forth in the Rates & Services Agreement and the I Do Hair and Makeup Artistry LLC./Go-Diva LLC. Policy Agreement and agree to forfeit any action of suit for any reason.     
 
*I am aware appointment times may be amended after agreement is in place at the discretion of I Do Hair and Makeup Artistry LLC./Go-Diva LLC    
 
* I confirm that by returning or emailing this form in part or in full it is considered an agreement between both parties (signed or unsigned) as approved by Go-Diva LLC and/or Margarita Potts. In addition, I hereby confirm that I understand and accept that by paying any amount including, but not limited to the 50% required deposit via cash, personal or cashier's check, money order, bank transfer, credit card or barter agreement as a deposit to reserve dates, times and/or appointments it will be considered an acknowledgement and acceptance to the terms in the Rates & Services Agreement and the I Do Hair and Makeup Artistry LLC/Go-Diva LLC Policy Agreement between both parties. The remaining payment is then due in full by the day of the ceremony.
